Payment Options
Self Pay
We are self pay or an out of network provider. This means that payment is due at the time of services. We do accept Health Savings Accounts (HSA) or Flex Spend from your employer. We also offer reduced wellness packages once you have completed Active / Acute relief care.
Initial Visit is $250 with follow ups ranging from $60 to $360. Click on “Book Appointments” for a more detailed list of services and pricing.
Insurance
We are not in network with and WE DO NOT bill insurance, Midcare, Tricare or Medicaid. We are happy to provide a superbill for you to submit to your insurance company for possible reimbursement after services have been received.
*For those not submitting for insurance reimbursement, we can provide an annual estimate of care.
Medicare and Secondary Insurance
This letter is to inform you of our office policy as related to Medicare. We do accept Medicare, as detailed by CMS, Medicare covers manual manipulation ONLY for ACUTE INJURY (We do not offer acute injury diagnosis or care for Medicare patients). Once you have reached maximum medical improve and/or maintenance care Medicare will no longer cover your adjustments. In some cases your secondary may cover some of your visit, but most of the time, if Medicare denies coverage the secondary will also deny. This includes Tricare, Blue Cross and Aetna Advantage, ect. We only offer maintenance/wellness care for Medicare patients and this service is not covered by Medicare.
The services NOT covered by Medicare are as follows:
X-rays
History and Physical ie Initial Evaluation
Maintenance Chiropractic Adjustments, Traction, Decompression, Massage, Trigger Point Release or any associated therapies.
